Adult Certification

Overview

This Certification Guide is intended for first time applicants and candidates for the ELSO Adult ECMO Certification (E-AEC). This guide outlines the requirements and processes necessary for E-AEC Certification. This guide details the importance of the educational and clinical experience requirements, in addition to the steps to successfully apply for E-AEC.

Purpose of Certification

Satisfactory completion of the criteria listed below will provide practitioners access to sit for the ELSO Adult ECMO Certification Exam. This exam is comprised of National Board of Medical Examiner (NBME)-style questions aimed at helping health professionals, across the continuum of care, enhance and demonstrate their clinical knowledge. ELSO undertook a multi-stage implementation process to ensure we can bring forward a solid knowledge assessment tool for the ECMO community. Our process included several rounds of psychometric analysis, on both expert and novice level ECMO clinicians.

The ELSO Adult ECMO Certification (E-AEC) is valid for a three (3) year period. The certification period is chosen to reflect the need for periodic reassessment while balancing the competing time demands for active practitioners. Recertification requirements are outlined in more detail in the “Current Certified Professionals” section on the Individual Certification page of our website.

E-AEC Certification Requirements & Documentation

To become an eligible candidate to sit for the E-AEC exam, ECMO practitioners must fulfill and demonstrate evidence of having met the following criteria:

Only courses that have been endorsed by ELSO officially or are run by ELSO can claim that they meet ELSO criteria. All other claims are self-proclaimed and may not be entirely accurate. ELSO publishes our learning objectives to make our standards available for anyone. However, this does not mean that the organizing entity for another course has met these criteria. Didactic Course (Step 1)


Practitioners are required to complete one didactic course that meets the ELSO Adult Learning objectives within the previous 15 years.

Documentation Requirements: Applicants must submit proof of the following

  • Course attendance (ex: completion certificate with course name, date, and candidate name)
  • Course agenda and learning objectives (for non-ELSO hosted or endorsed courses only, must clearly outline all learning objectives covered)

Simulation Lab (Step 2)


Practitioners are required to complete one high-fidelity ECMO simulation lab course that includes ELSO’s Adult Essential Simulation Scenarios:

  • Drainage Insufficiency
  • Air Embolism
  • Pump Failure
  • Membrane Lung Failure
  • Gas Failure

Documentation Requirements: Applicants must submit proof of the following

  • Proof of course attendance (ex: completion certificate with course date/location and candidate name)
  • Course agenda and learning objectives (for non-ELSO hosted courses only, must clearly outline that all essential simulations were completed in a high-fidelity environment)
     

Clinical Bedside Experience (Step 3)


Candidates must demonstrate completion of a minimum of 16-32 proctored hours (or 2 - 4 ECMO days) followed by a minimum of one 8-hour shift (or one day) every 8 weeks, for an average of 56 hours (or eight days) of ECMO patient care per year for a minimum of 6 months.

Please note: ECMO hours completed during fellowship training do not count toward this requirement. Step 3 is intended to assess post-training, independent practice.

Documentation Requirements: To verify that the clinical experience requirement has been met, the individual applying for E-AEC will need to submit a signed letter from the ECMO Director or Medical Director on the institution’s letterhead verifying the completion of the clinical experience requirement.

Submission Process

Timeline

E-AEC applications can be submitted at any time after the applicant meets criteria. We recommend not beginning the application until all steps have been completed and the applicant is ready to submit all materials. The application will timeout after 30 days and may need to be reset, application reset requests can be sent to ecmotraining@elso.org.

Submission Steps

Gather Documentation 

  • Collect all required documentation for didactic course and simulation lab
  • Receive a letter of completion of clinical experience requirement from ECMO Director, Medical Director or other appropriate personnel.
  • Ensure each document clearly shows the relevant details as previously described.

Access the E-AEC Certification Application

  • Access ELSO Academy via ELSO.org.
  • Select the E-AEC Application on the Dashboard, or from the Course Catalog.

Upload Documents 

  • Upload documentation as instructed within the E-AEC Application.

Complete Self-Attestation 

  • Review and complete the self-attestation statement within the applicant’s ELSO Academy account, and ensure all information is accurate.

Submit

  • Review submission for accuracy. 
  • Complete the submission by finishing the last page of the application. 

Processing Applications

Certification applications will be processed in the order in which they are received. Applicants will receive an email after their materials have been processed to confirm if any additional documentation is needed. Testing dates will become available on the candidate’s ELSO Academy Dashboard after application verification.

Fees

The E-AEC Application does not have a fee. A certification exam fee will be collected after the application is accepted and the candidate purchases their exam date. Payment is by credit card only via ELSO Academy.
 

E-AEC Exam

The E-AEC Exam is comprised of 120 National Board of Medical Examiner (NBME)-style questions aimed at helping health professionals, across the continuum of care, enhance and demonstrate their clinical knowledge. ELSO undertook a multi-stage implementation process to ensure we can bring forward a solid knowledge assessment tool for the ECMO community. Our process included several rounds of psychometric analysis, on both expert and novice level ECMO clinicians.  

Exam Proctoring

The E-AEC Exam is proctored using a software program that must be downloaded to your computer. It is the sole responsibility of the E-AEC Candidate to make sure that the proctoring software works with their system before they purchase the exam. The E-AEC Candidate is responsible for making sure that the proctoring software works with their system BEFORE they begin the proctored exam. ELSO recommends that all candidates use a personal laptop or computer to complete the E-AEC Exam. Institution-owned devices typically are programmed with software that may not be compatible with the exam. Mobile devices and tablets are not supported.  

Exam Scheduling

If the E-AEC application is accepted, the individual will be granted access to enroll in their desired exam period, organized by month. Exam periods run for two weeks beginning the second Tuesday of each month. Approximately 1 week prior to the exam period beginning, the enrolled individual will receive an automated email from the exam platform, Meazure Learning, granting access to sign up for an exam date and time. Testing dates and times must be scheduled at least 72 hours in advance to ensure proctor availability.  

E-AEC Certification Policies

  1. Certification Period
    1. Certification, initial or renewal, is valid for a period of three (3) years.
    2. Certification must be renewed before it is set to expire.
    3. E-AEC certified practitioners are required to know when their certification expires and are solely responsible for timely completion of all requirements for renewal, and timely submission of application materials.
    4. Failure to receive a notice or reminder of expiration does not excuse late or missing recertification application.
  2. Incomplete Applications
    1. Only complete applications will be reviewed. It is the sole responsibility of the individual applying to sit for the E-AEC certification exam to verify that their application is complete, includes all required documents as outlined in this document, and to respond to any corresponding communications from ELSO regarding their certification eligibility.
  3. Denied Certification Exam Applications
    1. E-AEC certification exam applications that are denied will be notified of the reason for denial, and request for additional documentation if applicable. This notice will include information on how to request reconsideration.
    2. If additional documentation is required, the individual applying to take the E-AEC certification exam should submit any requested documentation in a timely manner. Additional documentation may take up to 30 days to review and respond to.
  4. Failure of Exam and Retesting
    1. Individuals who fail to receive a Passing Score on the E-AEC certification exam, or who fail to complete the exam after they begin, may retake the exam provided the individual pays all applicable Exam Fees in accordance with the E-AEC Terms and Conditions and this document. Retesting may only occur after ELSO opens the next exam testing period following the close of the testing window in which the individual failed or did not complete the exam.
  5. Request for Reconsideration of Denied Certifications
    1. If certification is denied due to failure of the exam, the individual may submit a written request for reconsideration within 30 days of receiving the notification of their exam result.
    2. Request for reconsideration must include a detailed explanation of the grounds for reconsideration, including any supporting documentation or evidence not previously submitted.
    3. ELSO will review the submitted materials alongside the original application and any new evidence.
  6. Testing Accommodations
    1. If you have a disability that requires additional time or specific accommodations to take the Exam, please contact our team at ECMOtraining@elso.org prior to purchasing or downloading the Exam. ELSO must approve your request and make any adjustments before you sit for the Exam.
  7. Consequences of Rule Violations
    1. If you violate these or any ELSO rules, regulations, Terms, or Conditions for this Exam, you may be subject to an Unsatisfactory or Failed status on this Exam. For most ELSO courses, this will result in loss of academic credit and may be subject to future sanctions.
  8. Refund Policy
    1. All Exam purchases are FINAL. There will be no refunds for any reason. If you need to reschedule your exam to a later Exam period, your request must be made in writing to ECMOtraining@elso.org. All requests will be considered on a case-by-case basis.

Frequently Asked Questions (FAQs)

Can I recertify my E-AEC early?

You may begin uploading documents for your E-AEC recertification/renewal application up to three (3) months prior to its expiration; however, your certification will not be renewed until the calendar month it is due, and after ELSO reviews your submission. For example, if your E-AEC expires in October 2025, you may begin uploading recertification documents in August 2025, but your recertification will not be processed until October 2025.

What happens if I miss the recertification deadline?

If you miss the recertification deadline, your E-AEC will expire on the date posted on your certificate, potentially leading to the need to reapply for certification and meet all the initial certification requirements again, or the need to apply for reactivation of a lapsed certification, as outlined in this document.

It is essential to submit all recertification documentation and any applicable fees prior to the end of your certification's expiration month. It is the sole responsibility of the certified individual to know when their E-AEC is set to expire, and to apply for recertification prior to the expiration month.
